JOB FUNCTIONS:

Performs all direct care services in accordance with the Resident’s Care Plan
Performs necessary tasks to help the resident in ADL’s including, but not limited to, walking, toileting, changing, preparing for, and feeding meals with or without assistance
Performs tasks necessary to help the resident in maintaining personal hygiene such as, but not limited to, oral, foot, eye, and ear care
Participate in admission of residents as directed by Charge Nurse including, but not limited to greeting the resident and family and ensuring room and bed are ready.
Participate in transfer and discharge of residents, including, but not limited to pack and log
Assists with lifting, turning, moving, positioning, and transporting residents onto and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.
Perform rehab nursing procedures as instructed, including daily range or motion exercise. Record as indicated.
Assists in preparing the resident for physical examinations
Assists with the care of ostomies including, but not limited, colostomy and ileostomy, as per nurses’ instruction.
Care of the dying resident including care of the body and personal effects after death
Assists with maintaining bowel regularity; reports absence of bowel movements as per policy to nurse
Participates in and carries out job functions in the Nursing Department’s Program and activities as directed, in accordance with current Federal, State, and Local guidelines and regulations, accepted standards of practice, and as may be directed by the Facility Administrator or designee

QUALIFICATIONS:

Education: High School Diploma and/or equivalent, and successful completion of the nursing assistant course work and at least 3 clinical rotations accompanied by current PA Nurse’s Aide Certification and be listed in the Pennsylvania Aide Registry
Experience: Some nursing home experience is preferable, or at least six (6) months of full-time clinical experience.

JOB FUNCTIONS:

Carry out direct care to residents based on the care plan, nursing procedures, facility policy, and nursing standards of practice
Observes the resident for change in physical, social, or mental status; documents signs and symptoms and status of residents and related matters through oral/written reports to RN/RN Manager/Supervisor/Designee/Medical Staff and family as indicated
Participates in admission, transfer, and discharge including resident teaching as directed and documentation. Communicates response to teaching to RN/RN Manager/Supervisor/Designee
Administers clinical services such as medical treatments, diabetic testing, nutritional needs, and vital signs according to facility policies and procedures and nursing standards of practice
Administers medication and/or treatments as prescribed by the Medical Staff. Observe for side effects
Counts controlled substances ensuring that records and count are correct at all times, checks medication records, treatments records, and order sheets on residents to ascertain that orders written have been picked up and telephone orders have been countersigned and dated
Participate in the development and review of resident care plans for appropriate resident goals, problems, strengths, approaches, and revisions based on nursing needs
Assists and oversee distribution of food trays
Care of the dying resident, including care of the body and personal effects after death
Participates in and carries out job functions in the Nursing Department’s program and activities as directed, in accordance with Federal, State, and Local guidelines and regulations, accepted standards of practice and as may be directed by Registered Nurse and/or VP of Nursing or designee

QUALIFICATIONS:

Education: Graduate of an accredited school of nursing as an LPN accompanied by current LPN license in the State of Pennsylvania
Experience: Some geriatric experience is preferable, or at least six (6) months of full-time clinical experience.
Certifications: Current BLS/CPR training with card for verification, any other certifications required for working in specific specialty areas. (Optional)
