Moving a client from the bed to a wheelchair requires patience, communication, proper body mechanics, and close attention to safety. Before beginning, caregivers should review the client’s care plan, explain each step, lock the wheelchair brakes, move the footrests, and ensure the client is wearing non-slip footwear.

Accurate documentation is one of the most important responsibilities in healthcare. Every entry you make becomes part of the resident's permanent medical record and helps ensure safe, consistent, and high-quality care.

✅ DO:
• Chart promptly after providing care.
• Be accurate, factual, and objective.
• Document changes in a resident's condition immediately.
• Record only the care you personally provided.
• Use approved abbreviations and follow your facility's documentation policies.
• Sign or electronically authenticate every chart entry.

❌ DON'T:
• Chart care before it is completed.
• Guess or assume information.
• Copy another caregiver's documentation.
• Leave blank spaces or incomplete notes.
• Use white-out, erase, or alter documentation improperly.
• Include personal opinions, gossip, or emotional statements.
