The Intake Coordinator is responsible for managing all aspects of the SoCal intake process including communicating directly with participants, data entry, establishing and maintaining positive relationships with participants and community partners and referral sources, responding to requests and concerns, facilitating a daily overview of the program’s current standings of participants, and managing the eligibility processes.

We are looking to hire a dynamic and passionate Case Manager who is client-centered, community-focused, and is a self-starter. Providing individual/group support with linking, planning, advocating, and mentoring our SoCal participants. POSITION SUMMARY: Responsible for providing wrap-around case management support to participants including assessments, case noting, goal setting, reentry planning, progress monitoring, and group facilitation. In addition, the Case Manager is responsible for the onboarding, mentoring, and preparing the participants per cohort and will work with closely the Program Manager.
