CP Nassau is an independent, not-for-profit health agency serving over 1800 children and adults with cerebral palsy, developmental and other disabilities.

Adult Day Programs are year round programs serving individuals, 21 and older who have a developmental disability. They are comprehensive programs that focus on community integration, volunteerism, as well as working with individuals on maintaining and building upon current skills that promote independence, and add creativity to their daily lives.

Job Responsibilities:
• Drive individuals to and from program; to medical appointments, recreation outings, or other destinations as needed or requested.
• Assist in recreation activities.
• Work directly with residents to implement individual treatment plans, log progress and activities.
• Work with therapeutic and nursing staff to coordinate resident services.
• Provide direct care services for residents, such as toileting, feeding, dressing, sorting of laundry, etc.
• Attend in in-service training to ensure implementation of the formal and informal goal plan.
• Clean residents’ rooms, vacuum dust, change bed linens, make beds, empty trash buckets, clean bathrooms, do laundry, etc.
• Observe residents’ bodies on a daily basis, if required, and report all physical conditions including hair, nails, sores, insect bites, rashes, etc.
• Administer medications in accordance with AMAP guidelines.
• Contact families as needed.

• Work directly with individuals to implement Community Habilitation Plan activities including but not limited to activities of daily living (toileting, feeding, dressing, etc.), independent living skills (laundry, cleaning, money skills, etc.), and community integration skills (recreation, travel training, etc.).
• Document Community Habilitation activities on a daily and monthly basis.
• Maintain contact with families, supervisors and other parties as needed.
• Report situations/issues involving consumers to proper authorities.

The Community Based Day Habilitation is a year round program serving individuals, 21 and older that are developmentally disabled. Clinical services such as occupational therapy, physical therapy, speech therapy, and psychology services are available as part of the program.

Qualifications:
1. Clean and Valid NYS Driver’s License required
2. High School Diploma/GED preferred.
3. Must receive AMAP Certification within 6 months (Paid Training Provided)
4. HHA experience and one year experience with DD population preferred

Position Title:
Assistant Community Coach-2 Oceanside

Location: 421 Merrick Road, Oceanside, NY 11572

Key Responsibilities include, but not limited to:
• Implement and evaluate activities, which promote greater independence, inclusion, individuality and productivity for consumers.
• Assist consumers with all ADL needs including, but not limited to, feeding, transferring, lifting, toileting, dressing (coats), etc., as per consumer’s protocols.
• Provide AMAP services to assigned participants.
• Respond to parents/correspondents/advocates regarding consumers.
• Ensure needs of consumers are being met including but not limited to their safety and well-being.
• Accompany and drive consumers to and from locations for programs.
• Communicate with all clinical personnel to effectively deliver required services to consumers.
• Report any sickness, injury or seizure to medical personnel and supervisor or designee expeditiously.
• Ensure consumer appointments and schedules are adhered to.
• Ensure compliance with OPWDD/OPTS regulations and cpn policies/procedures.
• Responsible for doing bus duty.
• Drive consumers to and from program in AM and/or PM.
• Contact families as needed.
• Report situations/issues involving consumers to proper authorities.
• Incorporated Code of Ethics, Core Competencies and Empowerment Strategies in their daily professional responsibilities.

Physical Demands:
• The employee must be able to lift, bend and push more than 100 pounds.
• Employee is regularly required to stand and walk for extended periods.
• The employee is regularly required to use hand over hand assistance when necessary.
• The employee is occasionally exposed to moving mechanical parts and outside weather conditions.
• The employee is required to drive during inclement weather to community sights.

-Act as a resource to participants and families in regard to clinical programs by encouraging their participation in the therapeutic process through shared decision making and training.
-Train agency personnel in therapeutic carry-over activities and safety practices related to the therapeutic process.
-Maintain adequate participant records.
-Prepare required reports within the appropriate time frame
-Participate in agency opportunities for professional growth.

Physical Demands:
A physical therapist must have sufficient strength, balance and agility, to perform any necessary treatments, including lifting (we perform team lifts with two people or use the Hoyer/Invacare lift when needed).
