Department:
The Day Hab hub-site is located off the main campus in a store front in Oceanside. It is a comprehensive program, which focuses on community integration, volunteerism, as well as working with participant’s on maintaining and building upon current skills that promote independence, and add creativity to their daily lives.
The Community Based Day Habilitation is a year round program serving individuals, 21 and older that are developmentally disabled. Clinical services such as occupational therapy, physical therapy, speech therapy, and psychology services are available as part of the program.
Qualifications:
1. Clean and Valid NYS Driver’s License required
2. High School Diploma/GED preferred.
3. Must receive AMAP Certification within 6 months (Paid Training Provided)
4. HHA experience and one year experience with DD population preferred
Position Title:
Assistant Community Coach-2 Oceanside
Location: 421 Merrick Road, Oceanside, NY 11572
Key Responsibilities include, but not limited to:
• Implement and evaluate activities, which promote greater independence, inclusion, individuality and productivity for consumers.
• Assist consumers with all ADL needs including, but not limited to, feeding, transferring, lifting, toileting, dressing (coats), etc., as per consumer’s protocols.
• Provide AMAP services to assigned participants.
• Respond to parents/correspondents/advocates regarding consumers.
• Ensure needs of consumers are being met including but not limited to their safety and well-being.
• Accompany and drive consumers to and from locations for programs.
• Communicate with all clinical personnel to effectively deliver required services to consumers.
• Report any sickness, injury or seizure to medical personnel and supervisor or designee expeditiously.
• Ensure consumer appointments and schedules are adhered to.
• Ensure compliance with OPWDD/OPTS regulations and cpn policies/procedures.
• Responsible for doing bus duty.
• Drive consumers to and from program in AM and/or PM.
• Contact families as needed.
• Report situations/issues involving consumers to proper authorities.
• Incorporated Code of Ethics, Core Competencies and Empowerment Strategies in their daily professional responsibilities.
Physical Demands:
• The employee must be able to lift, bend and push more than 100 pounds.
• Employee is regularly required to stand and walk for extended periods.
• The employee is regularly required to use hand over hand assistance when necessary.
• The employee is occasionally exposed to moving mechanical parts and outside weather conditions.
• The employee is required to drive during inclement weather to community sights.
