Rev 4 isn't just a version number. It's the current standard for writing high-consequence procedures. If your program is still working to an older revision, the gap matters more than it looks.

PPA AP-907-005 Revision 4 reflects what the industry has learned about human performance: how people actually read, interpret, and act on a procedure under real conditions. Each revision sharpens that understanding. Rev 4 is where it stands today.

Here's why keeping current isn't just administrative housekeeping. A procedure written to an older standard can be technically correct and still carry the ambiguity, formatting, and level-of-detail issues that newer guidance was specifically designed to eliminate. The standard advanced because the understanding of how procedures fail advanced. Working to the current revision means your procedures reflect the current understanding of how to keep the person using them safe.

If your procedures were built to an earlier standard, it's worth asking when they were last measured against where the standard is now.

Every experienced team has a person everyone goes to for questions. The one who knows why the procedure says what it says, what the last audit flagged, and how the system actually behaves at the most critical times.

Then that person retires.

Most organizations don't feel the gap until they're standing in it. The knowledge was never written down, because the person who had it was always available. When they leave, decades of judgment leave with them, and what's left behind is a set of procedures nobody fully understands anymore.

This is the quiet risk in every high-hazard operation running on institutional memory. It doesn't show up on a metric. It shows up the first time something unusual happens when the person who would have known what to do is gone.

Capturing that expertise before it walks out the door is some of the most valuable work we do. We help organizations turn what lives in one person's head into procedures, training, and programs the whole team can rely on.

After enough years of investigating incidents, you stop being surprised by the mistakes and start being surprised by the conditions.

When a critical error happens in a high-hazard environment, the question shouldn't be "Why did they do that?" It should be "What made that possible?"

The answer we've found is that it almost always lives in one of three systems: Procedures, Training, or Leadership.

We call that the Performance Equation, and it's the framework underneath everything we write, build, and teach here.

That's why we designed a two-hour, self-guided online course you can take at your own pace.

When procedures, training, and leadership work as a single system, they become the foundation for safer, more reliable operations. When one is weak, the other two carry a risk they were never designed to hold.

"This is the most effective and well-delivered training I've experienced."

This review came from an Operations Specialist in oil and gas, an industry where every procedure step has to be clear, precise, and right the first time. It followed our Summer PPA Writer Certification Course, and it's exactly the outcome we build for. Here's the full quote:

"Procedure Solutions Academy delivers training the way high-risk industries actually need it. Their content is precise, thoughtfully designed, and grounded in human-factored principles that make learning stick. The clarity of instruction, the relevance of examples, and the seamless progression from concept to application are outstanding. This is the most effective and well-delivered training I've experienced."

Our commitment is to help professionals write human-factored procedures with clarity and confidence, so they can make a difference where it matters most.

We're hiring for a role you might not expect:

Not a procedure writer, not an SME... We're actually looking for a developer.

Specifically, someone who builds macro-enabled Microsoft Word templates. We need VBA, content controls, custom XML parts, and XML mapping. The unglamorous plumbing that lets a writer in a nuclear plant produce a compliant procedure without fighting the document.

This is an independent contractor role, project-based, remote, as-needed. You'd work with our team to turn client documentation standards into templates that function reliably.

Here's the thing: This person probably isn't reading our feed.

They're on a Word forum somewhere or on an automation team three degrees away from someone reading this right now.

So if you know that person — the one who's been writing Word VBA for fifteen or so years and is unreasonably good at it — send them this post!

Most procedure programs don't fail all at once. They drift.

It starts with a workaround here, an edit there, until what's on paper no longer matches what happens on the floor.

We can help.

We run end-to-end procedure projects: gap analysis, human-factored rewrite, and implementation.

You get documentation your workforce will actually follow and your regulators can trust.

If your procedures haven't been looked at with fresh eyes in years, that's the place to start.

Let's discuss how we can ensure you are up to standard and that your procedure program hasn't drifted to an unsafe place.

We're your Procedure Solution.

The organizations that see a real impact are the ones that also have our experts train their teams on the new standards being put in place.

Here's why that matters:
When the people who write your procedures are also the ones training your team on them, nothing gets lost in translation. There's no gap between the intent behind a step and how it's taught on the floor. Our experts don't have to hand your staff a document and walk away. They can stay to explain the why behind every procedure, the human performance thinking integrated into it, and the error traps they are designed to prevent. That's knowledge your trainers might not have.

That's the advantage of how we work.
Our subject matter experts have spent careers in nuclear, oil and gas, DOE, and utilities operations, not observing the work, but doing it. So we really can direct your training team how to carry the standard forward. We can also train your staff directly before implementation. Either way, the people delivering the material become the ones who know it thoroughly, because they helped build it.

Rewritten procedures sitting in binders don't drive lasting impact. The organizations that actually reduce errors are the ones that close the loop: new standards and expert training, from the people who understand not just what changed, but why.

That's the difference between compliance on paper and performance in the field.

If you want your procedures to evolve and your team isn't equipped to train them, that's a conversation worth having with us, because that gap is where risk lives. Let's close it together.

Day one of the Summer PPA Writer Certification Course is in the books, and what a start!

Nearly 50 professionals joined us yesterday to dig into the fundamentals that make procedures work: human performance modes, procedure and work instruction tools, the error traps that challenge even experienced writers, and the human performance tools that help writers get it right.

These aren't abstract concepts. Every objective in this course traces back to a real outcome: a task done safely, a step that couldn't be misread, an operation that ran as intended.

Special thanks to Steve McCord and Charles Schronick for kicking us off and leading this instruction well.

To everyone who showed up ready to invest in their craft: thank you. We've got a full week ahead, and another class right behind it. This is going to be a great one!

Rev 4 is the most significant update to the PPA procedure writing standard in years. So why does it matter?

Because the standard reflects how the best organizations in high-risk industries write procedures today — not five years ago. New topics, new material, new lessons, all refined to best practices. When the standard evolves, the work that depends on it should too.

Getting certified on Rev 4 means you're learning the current state of the craft, not operating on a previous version of it. Upgrading your certification ensures you walk away ready to write procedures that meet the industry where it is, and where it is headed.

The Summer PPA Writer Certification Course starts Tuesday and it's built entirely on the Rev 4 standard.
