Tryfacta is looking to hire 5 Social Workers for one for our clients in San Francisco.

This is a contract role that will run for 7+ Months and we can offer a very attractive hourly pay.

Included are the job details:

• Evaluates social, emotional and physical needs of clients by applying social work theories.
• Provides supportive counseling or crisis intervention to clients and/or their significant others, families
• Interviews clients, their families and significant others to obtain intake information; reviews financial status and screen eligibility for entitlement programs;
• Collaborates with interdisciplinary health care teams to treat and plan for the psychosocial and medical needs of patients by consulting with other providers and making referrals.
• Conducts mandatory reporting activities by completing reports (CPS, APS, elder abuse, and domestic violence reports) in specific timeframes and completing appropriate documents;
• Prepares records to document case activity including pertinent statistical reports, chart notes, correspondence, and other agency forms to monitor and assess client's progress;

Experience Required:

-EDUCATION: Possession of a Master's Degree in Social Work (MSW) from a school of social work fully accredited by the Council on Social Work Education (CSWE); AND

-EXPERIENCE: Six (6) months (1000 hours) of social work experience in a health care setting

Tryfacta is on the lookout for multiple Support Specialists who will be working at the Kentucky Transportation Cabinet (KYTC) and assisting in the Driver’s License & Real ID’s for supporting the is responsible for the issuance of driver's licenses, maintaining driver's records, and managing driver safety programs. '

The main focus of this position is to learn & implement the new digitized driver license issuance requirements along with the new state and federal rules regarding REAL ID and Commercial/National Driver Registration.
