More Details on Medicaid Rate Increases
Thank You to Governor Hochul and our Senators & Assembly Members


NYSVARA learned additional details on the Medicaid ambulance payment increases that will take place as a result of the $36.4 million in funding included in the New York Budget for state fiscal year from April 1, 2023 – September 2024. The funding has been approved by the legislature and enacted into law by the Governor.

Currently Medicaid pays ambulances the same amount for emergency and non-emergency transportation (if the dispatch comes from a 911 dispatch center, it is an emergency). These funds will increase the rate for emergency ambulance transportation.

There will no longer be county-specific prices on the Medicaid ambulance fee schedule. There will be a statewide price for BLS non-emergency, BLS emergency, ALS 1 emergency, and ALS 2 emergency.

Medicaid reimbursement rates are being revamping using Relative Value Units, or RVUs, that are based on time spent with the patient and the skills required. RVUs are an accepted concept and successfully used in Medicare payment systems.

The base rate – which is RVU 1.0 -- has been set at $195, which is an average of the County rates on the current Medicaid BLS fee schedule. This is the rate that will be paid for non-emergency or interfacility transports.

BLS emergency transports will be paid with an RVU multiplier of 1.28 or 128% of the base rate. ($195 x 1.28 = $249.60)

ALS 1 emergency transports will be paid with an RVU multiplier of 1.52 or 152% of the base rate. ($195 x 1.52 = $296.40)

ALS 2 emergency transports will be paid with an RVU multiplier of 2.2 or 220% of the base rate. ($195 x 2.2 = $429.00)

Specialty care transports (critical care) will be paid with an RVU multiplier of 2.6 or 260% of the base rate. ($195 x 2.6 = $507.00)


These changes will be implemented on or about July 1, 2023 and will be permanent. There is not an annual inflation amount.

There a few Counties that already have higher Medicaid rates on the current ambulance fee schedule. No counties will see a decrease, even if their rate is higher than the new rate.

These are the largest Medicaid rate increases in memory and they are being implemented without a phase-in period. NYSVARA made this a centerpiece of our 2023 legislative agenda and we are pleased with the fiscal support that this will provide to so many of our members that get paid less than the cost of delivering service to Medicaid members. We again express our gratitude to the State Legislature and Governor Hochul and thank the NYSVARA members who told their stories to members of the Legislature in support of higher Medicaid ambulance rates.
