Porter's point was simple: the CEO is the lead strategist. That work can't be handed off, and it doesn't happen in the margins.

You already know how to run the conversation. The question is whether you're listening to win it.

Most of us were trained in competitive listening. Track the objection. Line up the counterpoint. Get to the outcome. It works often enough that it never gets questioned.

But curiosity is the mechanical expression of humility. When you stay curious, you're telling the person across the table that you don't have a predetermined outcome. You can't know before you discover.

That's a choice you make in real time. Your brain will try to fill the gaps and finish their sentence for you. Active listening rejects the assumption and verifies at every stage. You're gathering information, not interpreting it.

What that protects is bigger than the deal. It protects the relationship. It builds trust. It says, "I care about you more than what I think you're saying."

The teams who get this right don't close harder. They discover better.

Most physician owners we talk with are not struggling with patient care. That part is working.

What is harder is the business behind the practice. Costs climb, reimbursements stay flat, administrative work expands, and the decisions that would actually move the organization forward keep waiting on people who are already full.

Revenue can grow while profitability, scalability, and transferability stay exactly where they were.

Here is what that looked like for one orthopedic and spine group. Synergy Health Partners, 31 providers, with underutilized surgery centers quietly limiting performance. Over eight weeks, their leadership team established surgeon block times, tightened ASC staffing, renegotiated supplier agreements, and built the reporting to hold it all accountable.

ASC utilization moved from 46% to 62%. Staff per case dropped from 24.6 to 13.4. Trailing 12-month revenue rose 25%. EBITDA rose 86%.

Their leaders made those calls. We brought the framework and the ex*****on support.

You built this. Every decision that got the company here was yours, and you made most of them with less information than you wanted.

This next decision deserves better than that.

Seven out of ten companies that go to market never close. Of the owners who do sell, most say afterward that they would have done something differently. The regret almost never comes from choosing the wrong door. It comes from choosing without seeing what was behind it.

You have three. Keep building. Sell. Pass it on. All three are legitimate, and which one is right depends on things only you can weigh: what you want the next decade to look like, what your family needs, what you are willing to trade for it.

What you should not have to guess at is the ground you are standing on. Where the business is today. What it is worth right now. What it would take to reach the number you already have in mind.

Put that in front of you and the judgment you have been using all along has something solid to work with.

You know how to make the call. Make it with the full picture.

You know your business better than any consultant ever will. That's not a limitation. That's your advantage.

Somewhere on your floor, there's a process that works because one of your people made it work. They built a workaround years ago and never wrote it down. You have the standing to go find it. Nobody else does.

Lean improves efficiency. Six Sigma reduces variation. Useful language, but the real move is simpler: you walk your process, you ask your people how they'd change it, and you listen to what comes back.

This isn't one more thing on your plate. It's the part only you can do. Your team already trusts you enough to tell you the truth. Go collect it.

Most service businesses can tell you their close rate. Far fewer can tell you what percentage of customers actually got the outcome they paid for.

That gap is not a measurement problem. It is a structural one. Sales owns the promise. Support owns the breakage. Nobody owns the middle, which is where the customer either succeeds or quietly decides not to renew.

You already know which accounts are thriving and which are drifting. You can feel it in the way calls go. The question is whether anyone in your organization is accountable for moving accounts from the second column to the first, on purpose, before the renewal conversation forces the issue.

That role has a name now. Whether you fill it or build the function yourself matters less than deciding someone owns it.
