After 34 years placing nurses in Midwest healthcare facilities, these three patterns keep coming up.

1. Waiting until a vacancy is critical before activating a staffing partner.
By the time a position is urgent, your options have narrowed and your costs have risen. Build your staffing partner relationships before you need them.

2. Evaluating agencies on rate alone.
The cheapest placement isn't the cheapest when it results in a poor fit, early termination, or a compliance gap. Credentialing rigor, specialty matching, and service reliability matter more than the invoice.

3. Underutilizing per diem for surge coverage.

The 2021 staffing crisis was reactive. Facilities scrambled, rates spiked overnight, and agencies with volume capacity won.

What we're seeing in 2026 is different — and in some ways more challenging.

Three things have shifted:

1. The burnout wave has stabilized, but the pipeline hasn't. Fewer new nurses are graduating than the system needs, and experienced staff departures continue.

2. Cost pressure is squeezing facilities back toward agency reluctance — even as permanent hiring remains difficult.

3. The contract market has bifurcated. Specialty-specific demand (ICU, OR, corrections) remains intense. General med-surg has softened. Facilities need precision, not volume.

What this means for Midwest hospital and facility administrators: the boutique agency model — built on relationships, not platforms — is actually better suited to this environment than the national aggregators.

We've been doing this for 34 years. This is not a new challenge for us — it's the environment we were built for.
