- 18 to 33 years old; MALE
- New Curriculum: Senior High School graduate
- Old Curriculum: High School graduate or;
- College undergraduate

1. (6) copies of β€œ1x1” clear and colored ID picture, white background.
2. (6) copies of β€œ2x2” clear and colored ID picture, white background.
3. (3) photocopies of SSS ID (back-to-back) or form with SS number.
4. (3) photocopies of TIN ID (back-to-back) or form with TIN number.
5. (3) photocopies of Philhealth ID (back-to-back) or form with Philhealth number.
6. (3) photocopies of Pag-Ibig ID (back-to-back) or form with Pag-Ibig number.
7. Original and (3) photocopies of Barangay Clearance.
8. Original and (3) photocopies of NBI Clearance.
9. PSA copy of:
- If married: (3) photocopies of Marriage Contract and Birth Certificate and (3) photocopies of each Birth Certificate/s of dependent/s (below 21 years old) children.
- If single: (3) photocopies of Birth Certificate and (3) photocopies of each Birth Certificate/s of dependent/s (below 21 years old) brother and/or sister, (above 60 years old) parent/s.
10. Original and (2) photocopies each of School records:
- For High School/Senior High School Graduate – Diploma and/or Form 137.
11. Original cedula.
12. Photocopy of vaccination card or vaccination certificate.
13. Photocopy of COE from previous work if available.
14. (3) photocopies of other valid IDs available.
