ដើម្បីបង្កើនការយល់ដឹងរបស់សមាជិកសហគមន៍ និងអ្នកពាក់ព័ន្ធនៅខេត្តកោះកុង អំពីដំណើរការ ការតាមដានដែលដឹកនាំដោយសហគមន៍ (CLM) និងតួនាទីរបស់ CLM ក្នុងការលើកកម្ពស់កម្មវិធីបង្ការមេរោគអេដស៍
ដើម្បីពង្រឹងសមត្ថភាពតំណាង DFoNPAM លើផ្នែកបច្ចេកទេសពាក់ព័ន្ធនឹងមេរោគអេដស៍ និងជំងឺអេដស៍, ការពង្រឹងប្រព័ន្ធសហគមន៍ (CSS) និងឧបករណ៍ CLM ដើម្បីអាចធ្វើការស្វែងរកការគាំទ្រ និងប្រមូលទិន្នន័យប្រកបដោយប្រសិទ្ធភាព|
ដើម្បីប្រមូល និងចែករំលែកបញ្ហា និងកង្វល់ពាក់ព័ន្ធដែលបានលើកឡើងដោយសមាជិកសហគមន៍ទៅកាន់អ្នកពាក់ព័ន្ធ ដើម្បីធានាថាបញ្ហាប្រឈមត្រូវបានដោះស្រាយ និងការទទួលខុសត្រូវត្រូវបានពង្រឹង

HACC សហការជាមួយវេទិកា FoNPAM និង DFoNPAM បានរៀបចំ កិច្ចប្រជុំផ្សព្វផ្សាយ និងលើកកម្ពស់ការយល់ដឹងអំពីការតាមដានដែលដឹកនាំដោយសហគមន៍ (CLM) នៅបន្ទប់ប្រជុំកាកបាទក្រហមកម្ពុជាខេត្តកោះកុង កាលពីថ្ងៃទី ១៩ ខែសីហា ឆ្នាំ២០២៦។
កិច្ចប្រជុំនេះមានអ្នកចូលរួមចំនួន ៥២នាក់ មកពីមន្ទីរសុខាភិបាល មន្ទីរអប់រំ យុវជន និងកីឡា មន្ទីរផែនការ មន្ទីរការងារ និងបណ្តុះបណ្តាលវិជ្ជាជីវៈ ក្រុមប្រឹក្សាឃុំ/សង្កាត់ អ្នកស្ម័គ្រចិត្តសុខភាពភូមិ (VHSG) មណ្ឌលសុខភាព ប៉ុស្តិ៍សុខភាព មន្ទីរពេទ្យ និងកាកបាទក្រហមកម្ពុជា។
ទិន្នន័យ CLM ដែលបានបង្ហាញក្នុងកិច្ចប្រជុំ បានបង្ហាញថា៖
🔹 ជាង ៩១% នៃអ្នកឆ្លើយតបចំនួន ៥៧២នាក់ បានរាយការណ៍ថា ពួកគេមិនធ្លាប់ទទួលបានការអប់រំអំពីមេរោគអេដស៍ និងជំងឺអេដស៍។
🔹 មានតែ ៦នាក់ ក្នុងចំណោម ១៦នាក់ ដែលធ្លាប់ជួបប្រទះករណីស្រោមអនាម័យបែក អាចទទួលបានសេវា PEP។
🔹 ៦៨% នៃអ្នកឆ្លើយតប មិនបានដឹងអំពី PrEP។
🔹 ១២នាក់ ក្នុងចំណោម ៦៦នាក់ បានរាយការណ៍ថា ពួកគេពាក់ព័ន្ធនឹង ការប្រើប្រាស់សារធាតុគ្រឿងញៀនរួមជាមួយសកម្មភាពផ្លូវភេទ (chems*x)។
លទ្ធផលទាំងនេះបានបង្ហាញពីតម្រូវការក្នុងការពង្រឹង សេវា PrEP និង PEP ពង្រីកការអប់រំអំពីមេរោគអេដស៍ និងជំងឺអេដស៍ដល់ ក្រុមប្រជាជនគន្លឹះ និងយុវជន ទាំងក្នុង និងក្រៅសាលារៀន ព្រមទាំងបង្កើនលទ្ធភាពទទួលបាន កិច្ចគាំពារសង្គម សម្រាប់អ្នករស់នៅជាមួយមេរោគអេដស៍ និងក្រុមប្រជាជនគន្លឹះ។
កិច្ចប្រជុំនេះត្រូវបានរៀបចំក្រោម គម្រោងពង្រឹងប្រព័ន្ធសហគមន៍ (CSS) ដែលគាំទ្រដោយមូលនិធិសកល (Global Fund)។ កិច្ចប្រជុំផ្ដល់ឱកាសសំខាន់មួយសម្រាប់តំណាងសហគមន៍ និងអ្នកទទួលខុសត្រូវក្នុងការផ្តល់សេវា និងអនុវត្តគោលនយោបាយ ដើម្បីពិភាក្សាអំពីបញ្ហាប្រឈម កំណត់អាទិភាព និងពង្រឹងភាពជាម្ចាស់នៅមូលដ្ឋានក្នុងការឆ្លើយតបនឹងមេរោគអេដស៍ និងជំងឺអេដស៍។
តាមរយៈ ការតាមដានដែលដឹកនាំដោយសហគមន៍ (CLM) សំឡេង និងភស្តុតាងពីសហគមន៍អាចត្រូវបានយកទៅពិភាក្សា ដើម្បីធ្វើការសម្រេចចិត្ត ដើម្បីជួយអ្នកពាក់ព័ន្ធកំណត់ចន្លោះប្រហោងនៃសេវា ពង្រឹងការទទួលខុសត្រូវ និងធានាថាសេវាពាក់ព័ន្ធនឹងមេរោគអេដស៍ឆ្លើយតបបានកាន់តែប្រសើរទៅនឹងតម្រូវការជាក់ស្តែងរបស់សហគមន៍។
ជាផ្នែកមួយនៃកិច្ចខិតខំប្រឹងប្រែងជាបន្តបន្ទាប់ ដើម្បីពង្រឹងការតាមដានដែលដឹកនាំដោយសហគមន៍ អេច.អេ.ស៊ី.ស៊ី សហការជាមួយវេទិកា ហ្វុនប៉ាម ឌីហ្វុនប៉ាម តែងតែរៀបចំវេទិកា CLM នៅក្នុងខេត្តគោលដៅចំនួន ១២ រួមមាន៖ ខេត្តកំពង់ឆ្នាំង កំពង់ធំ ព្រះសីហនុ កំពត សៀមរាប ស្វាយរៀង តាកែវ កោះកុង បាត់ដំបង បន្ទាយមានជ័យ រាជធានីភ្នំពេញ និងកំពង់ចាម។
To increase understanding among community members and stakeholders in Koh Kong about the CLM process and its role in enhancing HIV prevention programs,
To improve the capacity of DFoNPAM representatives on the technical aspects of HIV and AIDS, Community System Strengthening (CSS), and CLM tools to enable effective advocacy and data collection,
To gather and share relevant issues from community members with key stakeholders, ensuring that challenges are addressed and accountability is strengthened,
To increase collaboration and partnership among local authorities, health service providers, and other stakeholders within the HACC, FoNPAM, and DFoNPAM frameworks,
HACC, with FoNPAM and DFoNPAM, conducted a Community-Led Monitoring (CLM) Awareness Meeting at the Cambodian Red Cross meeting room in Koh Kong Province on 19 August 2026.
52 participants representing the Department of Health, Department of Education, Youth and Sports, Department of Planning, Department of Labour and Vocational Training, Commune Councils, VHSGs, health centres, health posts, hospitals, and the Cambodian Red Cross actively participated.
The CLM data presented during the meeting highlighted several important gaps:
🔹 91%+ of 572 respondents reported that they had not received HIV and AIDS education.
🔹 Only 6 of 16 respondents who had experienced condom breakage were able to access PEP services.
🔹 68% of respondents were not aware of PrEP.
🔹 12 of 66 respondents reported being involved in chems*x.
These findings highlighted the need to strengthen PrEP and PEP services, expand HIV and AIDS education among key populations and young people, both in and out of school, and improve access to national social protection schemes for people living with HIV and key populations.
The meeting was conducted under the Global Fund-supported CSS project. The meeting also provided an important platform for community representatives and duty bearers to discuss challenges, identify priorities, and strengthen local ownership of the HIV response.
Through Community-Led Monitoring, community voices and evidence can be brought directly into dialogue and decision-making, helping stakeholders identify service gaps, strengthen accountability, and ensure that HIV services better respond to the needs of communities.
As part of its ongoing efforts to strengthen community-led monitoring, HACC, in collaboration with FoNPAM and DFoNPAM, regularly organizes CLM forums across 12 targeted provinces: Kampong Chhnang, Kampong Thom, Preah Sihanouk, Kampot, Siem Reap, Svay Rieng, Takeo, Koh Kong, Battambang, Banteay Meanchey, Phnom Penh, and Kampong Cham.
