The Purpose of this role

As a full time Support Worker you will flexibly support each service user within their own home and in the wider community, responding to their individual needs and aspirations as well as assisting to maximise their independence whilst responding to all opportunities presented to the service user during their day to day life.

You will need to support and assist with all aspects of daily living including personal care and environment management, as well as with leisure, recreational, educational and employment based activities.

You will need to support service users to become a valued and active member of their local community.

Main Responsibilities:

• The direct delivery of care to our service users, in congruence with the six values; these are choice, fulfilment, privacy, dignity, rights, and independence.
• To work with other staff in a manner that promotes team-building, within the scope of company policy & procedures

Principal Duties and Responsibilities:

• Arrive punctually for duty and leave shift at the appointed time.
• To undertake all training as required by the company
• To provide support towards planning a well-structured programme of leisure activities and social skills training for each individual.
• Ensure the care plan is followed to promote the service user’s intended outcomes.
• Complete the daily entries as an accurate record and assist the line manager in evaluating the outcomes, and where necessary in conjunction with the service user & line manager make any adjustments. To maintain files & other records.
• Ensure the risk assessment is followed and alert the line manager to any concerns.
• Dispense medication if and when required according to medical instructions.
• Help the service users make any necessary medical appointments and attend with them as required.
• Promote the service user to enhance & develop their daily living skills e.g. helping them to choose & buy clothes, wash & iron garments, help menu plan, and assist with preparation or cooking where able to do so.
• Provide support to service users with their personal care as & when required.
• Assist service users to manage their personal finances & budgeting in line with the company’s policies & procedures.
• Support the service user to maintain their tenancy agreement, and where necessary support them to deal with landlords, workmen, and if necessary, any neighbourhood disputes.
• Offer assistance to access community-based activities, leisure time, education and supported employment.
• Protect the welfare of service users, and follow the company’s safeguarding procedure, reporting any acts of abuse or poor practice.
• Help service users to maintain & keep their environment clean.
• Act as a key worker to designated service users.
• To travel between properties and other locations such as Head Office as needed

Job Purpose:

• Lead the staff team in providing a high quality, effective service within the scope of company policy & procedures.
• To provide a homely, therapeutic & safe environment that meets the patient’s individual needs and facilitates integration with the local community.
• To work with other staff in a manner that promotes team-building, within the scope of company policy & procedures

Main Responsibilities:

• Ensure daily tasks are allocated appropriately.
• The implementation and provision of patient care for patients aged 18-65 presenting learning disabilities, mental health and challenging behaviour in accordance with the Health and Social Care Act 2008, Mental Health Act 1983, as amended in 2007, the company’s six core values, and company procedures.

Principal Duties and Responsibilities:

• Encompass a caring, supportive attitude and listening to service users about their concerns or feelings about any difficulties they may be facing.
• Writing review reports, MDT notes, risk assessments, care plans or any other reports as required.
• Being involved in the admission process of new service users.
• Assist in providing personal care to the service users.
• Maintain the standards of service user care and rehabilitation in accordance with recommendations, guidelines, and support plans from the Clinical Services • Team (psychiatry, psychology, nursing, occupational therapy, speech & language therapy and sessional therapists).
• To manage, monitor, and intervene where necessary in any situation where using approved Therapeutic Management of Aggression (TMA) techniques are required.
• Support during induction programmes and carrying out supervision of staff as and when requested to do so by the management team.
• Carrying out domestic duties including cleaning, washing, cooking as and when required to do so.
• Ensure that the home and living areas are kept presentable and care clean, tidy and safe.
• Ensure that each service users health needs are met.
• Liaise effectively and professionally with other relevant professionals, both internal and external; such as the GP, social worker or dentist.
• Awareness and implementation of all policies and procedures.
• Attend multi-disciplinary, mental health review, team and CPA meetings when necessary.
• To participate in all mandatory training and any other training as required.
• Carrying out any other delegated tasks given from the Registered Manager, and performing any other such duties as may be reasonably required/expected.
Ensure all documentation is kept up to date, and accurate.
• Promote the Accedo Group policies, procedures and philosophy.
• To assist the Manager in the assessment of individual staff and team training needs, which include planning and co-ordinating the provision of such training.
• To ensure a high standard of communication between staff, and arrange regular staff meetings, key worker reports, supervisions and appraisals.
• To wear ID badges at all times when on duty.

Financial Management:

• Responsibility for petty cash and patient cash.
• Ensure there are no amendments to invoices or fees without the written consent of the Director of Finance.
• Ensure that no agreement of expenditure is incurred on behalf of the company outside budgetary controls without the express consent of a Director.

Extra duties:

• Responsible for ordering, and ensuring the correct administration, control, storage and recording of medication.

We offer all staff a full training package.

This includes both E learning, live courses and in-house training and mentorship based on subjects that are relevant to the individuals we support.

We offer all of the standard as part of our induction such as first aid, safeguarding, MCA/DOLS, health & safety, medication, COSHH and infection control.

Our further training courses and bespoke tailored training packages include subjects such as , ’s, , , report writing and risk assessment. We discuss training needs with all staff in supervision and welcome ideas from all of our employees about other training options they would like to explore.
