•To carry out On call duties, overnight and at weekends, on a rota basis as determined by the company, when On call, provide telephone support and assistance to Care Support Workers and Service Users.

•Liaise with local authorities and private client families/representatives.

•To implement a high quality care services through the effective matching of Service user needs with suitably qualified and experienced care workers.

•To monitor and respond to any changes which may impact on rota including sickness absence, holidays, trainings, cancelation and other unforeseen circumstances..

•Ensure staff are aware of health and wellbeing policies

•Process changes to customer care and support plans.

•Prompt response to customer queries and phone calls.

• Take responsibility for new referrals from private customers and social services teams, ensuring full information is received and appropriate care planning is undertaken.

•Assist Registered Manager with identification of further business opportunities outside of client base.

•Organize the Care team, training and wellbeing policies.

•Schedule all new weekly MDT meetings, manage the meeting agenda items, ensuring that all new referrals are identified ,and information circulated to team members.

Our ideal Candidate will have

• full UK drivers licence, have access to a car.

•Previous experience with rota planning and allocating staff.

•NVQ/QCF Level 3 Health and Social care

•2 years minimum hands-on Care Experience.

•Ability to work autonomously.

•Good Communicator and team player

•Good computer skills

Roles and Responsibilities

Allocation of care workers to Care Homes around South Yorkshire.
Take responsibility for new referrals from private customers . Assist Our Manager with identification of further business opportunities outside of current client base.
Liaise as necessary with partner professionals and organisations (e.g. social workers, GPS, nurses) in respect of developing and delivering holistic, person-centred packages of care
Dealing with queries from clients and families in a calm and professional manner.
Organising and supporting with Staff Supervisions and Appraisals
Attending and participating in Staff team meetings.

CURRENT KEY:
➡️To call and expand client base; ensuring that the introductory pack is sent in the first instance.
➡️To complete all Carer’s files, check compliance and follow up on missing documents
➡️To carry out carer interviews

BASIC JOB DESCRIPTION

· To clearly set out the expectations for each shift and direct the team accordingly.
➡️To generate, manage and monitor the carers rotas, ensuring that clients calls are scheduled as close to their requirements, requests and preferences as possible.
➡️To ensure care calls are allocated to staff in a way to maximise efficiency, taking into consideration travel time, required breaks and availability.

➡️To work with the Registered Manager and other senior team members to facilitate new clients, where possible.

➡️To monitor staff levels and availability against the requirements of clients, prompting changes where required.

➡️To monitor and respond to any changes which may impact on rotas including sickness absence, holidays, training, cancellations, and other unforeseen circumstances.

➡️Ensuring that clients are kept up to date with any changes to their schedule or their usual Care Support Workers.

➡️Line management responsibility of Care Co-ordinators, including performance management, appraisals, supervisions, training and development, discipline, grievance and all other management duties.

➡️To be on call for emergencies, out of hours service, Monday through to Sunday on a rotational basis.
SALARY FROM £22000-£24:000 PER ANNUM.
