Have an understanding of Health and Social care in accordance to recruitment agencies
Find cover for any shifts which may be outstanding, prioritising the urgent ones
Making sure Clients & candidates are kept updated throughout the weekend
Efficiently deal with client request, monitoring clear and effective outcomes
Cover client requirements and match our service/staff to meet their needs
Develop appropriate relationships with clients and temporary staff
Management of temporary staff to ensure service delivery level is met
Ensure a detailed handover is prepared ready for 9am on Monday from the weekends activity.

You will support our clients to live an independent life whilst supporting them to access the local community, build relationships with their families and friends and live a happy and meaningful life.

About the Outreach Support Worker role:
Our Outreach workers in support people with long-term mental health problems. They help them adapt to ordinary life within the community by developing coping skills within a framework of positive consultation and joint planning.

They will work closely with all members of the community team to enable their clients to continue living in the community with comfort and dignity. The overall aim is to improve the quality of life of people who have mental health difficulties and prevent inappropriate admissions to hospital.


On a day-to-day basis the outreach worker will:
· attend the homes of identified individuals/families to carry out duties and tasks as specified in the care plan designed to enable a good degree of independent living - for example, support with correspondence, benefits, housing repair issues and advice to appropriate support such as referring to advocacy services or citizens advice.

· listen and give consideration to what the client would like to do and help them as best they can;

· identify deterioration in clients which will enable action to be taken to help them, for example with physical care such as bathing, cooking and so on. Refer to appropriate team.

· Ensure a safe environment for those who need long-term supervision and support;

· note any significant changes in clients health, outlook and behaviour and refer upwards for advice about what action to take;

· assist in any assessment procedures;

· provide regular verbal and written reports to the senior outreach worker/manager and in particular raise any issues about any alteration in the client's family circumstances;

· manage the induction process of newly appointed outreach staff.

· To use a range of evidence based tools and techniques for motivating clients.

To do the job well you would need:
· an understanding of mental health issues;

· good interpersonal skills;

· ability to communicate effectively in writing and orally;

· capable of maintaining appropriate written records;

· awareness of confidentiality;

· to be able to work on your own initiative as well as part of a team;

· awareness of health and safety issues for others and self;

· a caring nature;

· be able to follow instructions as part of a planned program of care;

· awareness of equalities legislation.


Due to it being an Outreach service covering a range of areas in Warrington, the successful applicant needs to hold a full driving license and have access to a vehicle with business insurance. You will be reimbursed for travel time and mileage.
