Care Coordinator Role

We at Take 4 Care are looking for a kind, dedicated and caring individual to join our team, both in the office as well as out providing care for our clients. The individual for this role will need to have a full UK driving licence and access to their own car, be willing to work as part of a team and cover care calls in the community alongside our care team. Key responsibilities will include implementing a domiciliary care service for people living in independence through timely interventions and a coordinated approach, liaising with other professionals in the best interest for clients, liaising with family members of clients and working alongside our team to provide the best person-centred care.

Job Type: Permanent

Schedules: 37.5hrs/week, 1week in 3 responsibility of on-call Phone. You will be required to cover care calls over varied shift patterns and may occasionally include weekend and evening work.

Duties: Coordinating service need & staff availability,
support process and systems to create safe compliant service, support Registered Manager

Salary: £26,500 plus on call

Benefits: Paid mileage, pension scheme, weekly pay, free PPE
