Key Responsibilities:

• Manage a caseload of adult service users a GPSC setting.
• Collaborate closely with key partners like GPs and other social care professionals to achieve the best health and wellbeing outcomes for individuals.
• Work with service users towards detoxes both community and in patient as well as rehabilitation centres.
• Work collaboratively with key partners and colleagues to achieve the best health and well-being outcomes for individuals.
