The Role

AS a Care Coordinator, you will be working as part of a great professional team delivering a high-quality personal centred care service, you will report to the Care Manager and will take responsibility for the day-to-day management of operations in the field. You will be responsible for staffing, undertake client reviews and assessments & will provide leadership & supervision to staff across all areas of operation and On calls

RESPONSIBILITIES

Take responsibility for the management and creation of rotas.

To oversee the running of the service in the Care Manager and Registered Manager’s absence

Work along side the Care Manager in delivering high quality care

Undertake client assessments, reviews & produce client care plans in line with the company policies & procedures. Facilitate a professional & personal care service which meets the client’s needs.

Liaise with clients, families, and professionals bodies to manage care needs and staff deployment

Managing staff rostering and assisting in the day to day operation of the business including staff registrations.

Supporting the care team to enable them to maximise their potential

Covering the out of hours to support the team and deliver care when required - Liaise with the Care Manger and the

Registered Manager to continually improve the service

Previous experience as a Care Coordinator is essential
Full UK driving licence and must own a car.

Responsibilities :*

As a Care Manager you would working with the Registered Manager and you will be supported by the Care coordinator and the team leaders, you will also be responsible for ensuring the service meets Health and Social Care Standard required by the relevant bodies. These include, CQC, Social Services and the City council.

You would:

Be responsibly for the health and safety of the people using the service and the staff providing the support
Be responsible for promoting the rights of people using the service and safeguarding them against harm
Be involve in the safe recruitment of staff and managing and developing your team, arranging training and providing regular supervision
Manage budgets
Develop policies and procedures to reflect and meet the National Healthcare standards
Ensuring any care delivered, administration of medication, moving and handling is done within the regulations
Get full knowledge of the people using the service, their individual support needs to your team provide the correct level of support
Provide information and advice to the people using the services, their families and staffs
Create opportunity for people using the service to contribute to their local community and access local service
develop good working relationships with carers, families, health service professionals and other agencies
