**Requirements for Poland**
Please make 3 photocopies of all your documents to avoid conflicts

1.Original and photocopy of All Old passport with visa stamp(no need to photocopy of visa stamps
2.Original and photocooy New passport (with visa stamp if you have)no need to photocopy the visa stamps
3.Original and Photocopy of your Certificate of employment for all job experience
4.Original and Photocopy of your Skills Certificate (optional)
5. Schengen id picture with white background/wear dark color with collar (taken within the last 6 months)
6.Original and photocopy of your School Credentials (HS GRAD/UNDER GRAD/ COLLEGE GRADUATE) Need to Apostille in DFA
7. Original and Photocopy of your Philippine ID such as UMID,Voter’s ID or any Valid Philippine id
8. NBI TRAVEL ABROAD need to Apostille in DFA
9. Bank Certificate/SOA (atleast 50kphp in your bank account for the last 6months)
10. PEOS certificate
11. Vaccination certificate
12. PSA Birth certificate
13. Bring 5kphp for your medical examination upon report to our partnership Agency in Philippines
14. Bring atleast 5kphp for the BLS fee for visa appointment upon report to our partnerhip Agency in Philippines
15. Pagibig # and SSS #

Reminder:
All photocopies must be in A4 size
