Matthew has been engaged with Elevate You Australia for 2.5 years with the goal to reach his career goals and achieve his bachelor of science with Melbourne University. EYA are so proud of the work he has achieved, we have assisted him in successfully managing adult learning requirements, overcoming the barriers he had in social settings, self direction, advocacy, and autonomy, along with building his capacity in accountability, responsibility, reliability, resilience, confidence and communication.
We helped Matthew gain and keep employment as a disability advisory panel member at the University of Melbourne, secure a summer internship role with the Department of Energy, Environment and Climate Action, where he contributed to public guidance documents such as the Guidance for drone operators: avoiding impacts on wildlife and relevant authorisations which was published on Friday on two seperate sites - Victorian Government site and the Casa Website https://content.vic.gov.au/media/1019431
Matthew will complete his Bachelor of Science (Marine Biology) in one more semester!

Elevate You Australia (EYA) are hiring!

EYA have offices in Dandenong, Lilydale, Box Hill, Sunshine , and Geelong and are currently expanding our Dandenong and Sunshine teams - We are seeking 3 full-time, highly motivated, and compassionate Disability Employment Case Manager/Mentor/Coach/Youth Worker to provide support to individuals with National Disability Insurance Scheme (NDIS) School Leaver Employment Supports (SLES), Employment assistance, or Finding and keeping a job funding.

As an EYA Case manager, you will be responsible for developing and implementing individualised support plans for participants, transitioning from high school to future adult independent life. There is a strong focus on coordinating and monitoring their progress, and ensuring they achieve their employment and vocational goals.

Location – Dandenong, Box Hill, and Sunshine offices.

Main duties, responsibilities, Core objectives, and responsibilities include:
- Mentoring clients to encourage a level of competence and independence with their employment and training goals.
- Conduct initial assessments to determine participant's needs, abilities, and goals.
- Develop and implement individualised support plans that are tailored to each participant's specific needs and goals.
- Work collaboratively with participants, their families, support workers, and other stakeholders to ensure effective delivery of services.
- Provide ongoing guidance and coaching to participants and their families throughout their service.
- Identify and address any barriers to employment or independent living and develop strategies to overcome these barriers.
- Maintain and monitor accurate and up-to-date records of participant progress and any relevant documentation required by the NDIS.
- Ensure compliance with NDIS guidelines, standards, and requirements, involving report completion and submission where required for funding purposes.
- Assisting with group activities or excursions – generating, facilitating, and adapting the activity.
- Liaise with various stakeholders to raise awareness and advocate for the participants needs
- Engagement and referral to beneficial services which will enhance the participants life and goals.
- Develop training and mentoring sessions to develop soft skills such as financial knowledge, independent living skills, employment nuances and social etiquettes etc.

About You/Requirements:
Bachelor's degree in a relevant field, such as Community Services, psychology, an Allied Health/Teaching related field, disability studies, or social work is preferred.

A minimum of Certificate IV in Mental Health, Disability, Health, Therapies, or Case Management will also be considered.

A minimum of 2-3 years of experience in disability support services or case management, with experience working with NDIS participants preferred.

Experience in SLES highly favoured.

Experience and proven knowledge of working with diverse cultures and communities, including Deaf, Autism, and indigenous.

You must hold a current driver's license, working with children check and NDIS workers screening check.

Excellent communication skills, with the ability to develop rapport and build relationships with participants, families, and other stakeholders.

Ability to develop and implement individualised support plans that are tailored to each participant's needs and goals.

Strong organisational skills, with the ability to manage multiple priorities and competing demands.

Proficiency in Microsoft Office and database management software.

Elevate You Australia has ranked 3rd out of 288 providers in Australia delivering School Leavers Employment Supports! The report that was released a couple of weeks ago shows the results of 2023-2024 (it's ridiculous in this day and age that the NDIS can't produce more current results, but regardless it's a reflection!) this report also doesn't reflect the amount of people we have in open employment who we are still supporting until they are truly independently working. So the numbers of placements we achieve are in reality much higher, but to be 3rd in Australia is something I'm very proud of and so grateful for the amazing staff (past and present) we have working endlessly on our participants with the same passion I have. Go team EYA! 💪🏼
