Hiring Hiring Hiring at Alice Springs - Health/Activities Care Coordinator or Senior Support Support.
Full time/Part Time position.
1. Case management
2. Providing a high level of support to help people access and implement the supports available to them in their plan.
3. Being a central point of contact for the people you support and service providers to ensure ongoing continuity and quality of tailored services.
4. Administration coordination including researching, updating files, budget templates, document management and maintenance.
5. Facilitating and strengthening relationships that are central to each person you assist.
6. Ensuring that goals and outcomes are met, providing regular reports on success as they relate to each individual’s goals and aspirations.
7. Assisting with NDIS reviews enabling each person the opportunity to ensure that their goals and aspirations are achieved as they progress.
8. Building and maintaining meaningful work relationships with your co workers, wider services and other networks so that together there is a connected circle of care for the person you support as well as yourself.
