We are looking for a Case Manager with experience to join us on a permanent full time basis. Covering a 2 hour travel radius from the centre of Leeds. You will be providing a Case Management service to our clients that is devoted to the coordination, rehabilitation, care and support of people with complex clinical needs. When delivering Case Management, Resurgence Case Management keeps the client at the centre of all activities, we have an ethos that maintains a holistic approach and strives to thoroughly understand the needs and circumstances of each individual and their family; so that effective solutions can be pursued that will work for their individual and unique situation.
You will need to have a professional registration (OT, Nurse, Physio, Psychologist, Social Worker, speech therapist....) and will need to remain compliant with all of the requirements of your relevant registration body (HCPC, NMC...)
We are a supportive and positive employer, you well get extensive induction and ongoing support, we offer 8 weeks paid holiday per year and a 35 hour a week full time contract with an annual salary over £40,000 to be agreed depending on experience.
