Job Summary

The Support Worker will provide high-quality, person-centred care and support to adults with learning disabilities and complex needs attending the service. The role focuses on promoting independence, social inclusion, and wellbeing through structured day activities and community engagement.

Key Responsibilities
Support service users in daily activities within the centre and community
Assist with personal care where required, maintaining dignity and respect
Facilitate group activities such as arts, crafts, life skills, and recreation
Support individuals on outings (e.g. shopping, leisure, appointments)
Follow individual care plans and contribute to person-centred reviews
Promote independence, choice, and inclusion at all times
Build positive, professional relationships with service users, families, and colleagues
Observe, record, and report changes in service users’ needs or behaviours

Essential Criteria

Hours: 35 hours per week (varied shifts)

We are seeking an Intensive Support Worker to join a dedicated team supporting individuals experiencing homelessness and other complex challenges. This role involves working with people who may be facing multiple issues including unstable housing, mental ill-health, substance misuse, offending behaviour, physical health concerns, and trauma.

The Role

Provide ongoing case management and support to a caseload of individuals with complex needs.
Build trusted relationships and develop personalised support plans.
Work closely with multi-disciplinary teams and partner agencies to coordinate wraparound support.
Support individuals to access services, sustain accommodation, and make positive lifestyle changes.
Monitor progress, review outcomes, and help overcome barriers to engagement.
Advocate for service users and contribute to achieving positive long-term outcomes.

Essential Requirements

Level 3 qualification in Health & Social Care or a related social care field.
Minimum of 2 years' paid experience working with vulnerable groups.
Full driving licence and access to a vehicle insured for business use.
Willingness to travel regularly within the local area.
